Counties in Health Horizons Service Area

Health Horizons of East Texas, Inc. was established in 1990 as a non-profit community based organization to provide HIV/AIDS care and related services in a number of counties in rural East Texas. The counties in the service area are: Angelina, Houston, Jasper, Nacogdoches, Newton, Polk, Sabine, San Augustine, San Jacinto, Shelby, Trinity and Tyler.

Mission

Health Horizons’ mission is to empower and serve the people of Deep East Texas with needs related to HIV/AIDS and other health issues. The agency collaborates with other organizations to help its clients improve their quality of life by offering a wide array of services including educational awareness. In addition Health Horizons strives to provide as many service locally as possible so clients do not have to travel long distances to Galveston or Houston to receive them.

Health Horizons currently has an active case load of approximately 250 clients. It has been successful in educating residents in its service area about HIV and its services through the media and educational presentations. The agency has overcome significant hurdles to become what it is today. Providing services confidently through its 20 staff and a host of volunteers, Health Horizons has gained respect and acceptance in the communities which are served. The agency frequently receives referrals for services on behalf of persons with HIV/AIDS from social workers, medical professionals and other service organizations.
